TRT Safety: What Current Research Actually Says About Long-Term Use

Testosterone replacement therapy has spent years caught between two extreme narratives.

One side presents TRT as inherently dangerous, with unavoidable risks to the heart, prostate, and overall health. The other treats it like a harmless upgrade that almost any man can use indefinitely.

Current research supports neither position.

The evidence available in 2026 is more reassuring than it was a decade ago, especially regarding major cardiovascular events and short-to-medium-term prostate outcomes in appropriately selected men. At the same time, TRT still has meaningful risks. It can raise hematocrit and blood pressure, suppress fertility, affect sleep apnea, and produce other side effects that require ongoing clinical attention.

The most accurate conclusion about TRT long-term safety is not that testosterone is universally safe or unsafe. Safety depends on who receives it, why it is prescribed, the level achieved, the treatment method, and whether the patient is monitored after therapy begins.

What Does โ€œLong-Term Safetyโ€ Actually Mean?

The phrase โ€œlong-termโ€ is often used loosely in hormone discussions.

A study lasting several months can identify common side effects, but it cannot answer questions about outcomes that may take years to develop. A trial following patients for approximately three years offers stronger evidence, but it still does not establish what happens after 10, 20, or 30 years of treatment.

Randomized controlled trials are useful because they reduce many sources of bias. However, they are expensive and rarely continue for decades. Observational studies can follow patients for longer, but the men who receive treatment may differ from untreated men in ways that affect the results.

This means TRT research in 2026 can answer some safety questions with growing confidence while leaving others open.

It can tell us more about major cardiovascular events over several years, changes in blood counts, blood pressure, and short-term prostate outcomes. It is less definitive about lifetime cardiovascular exposure, very long-term prostate cancer risk, and treatment beginning in younger men and continuing for decades.

What the TRAVERSE Trial Found About Heart Risk

The most influential modern cardiovascular study is the TRAVERSE trial, published in 2023.

The trial included more than 5,200 men between ages 45 and 80 who had symptoms of hypogonadism, repeatedly low testosterone levels, and either existing cardiovascular disease or a high cardiovascular risk. Participants received testosterone gel or placebo and were followed for approximately 33 months on average.

Testosterone therapy was not associated with a higher rate of the trialโ€™s primary cardiovascular outcome, which included cardiovascular death, nonfatal heart attack, and nonfatal stroke. In statistical terms, testosterone was noninferior to placebo for major adverse cardiovascular events.

That result directly challenged earlier fears that medically prescribed TRT necessarily increases heart attack or stroke risk.

However, the trial did not show that TRT protects the heart. It also identified higher rates of atrial fibrillation, acute kidney injury, and pulmonary embolism in the testosterone group. Those findings require additional study and should not be ignored simply because the primary cardiovascular endpoint was reassuring.

The correct interpretation is narrower: in the population studied, TRT did not meaningfully increase the combined risk of cardiovascular death, heart attack, or stroke over the study period. That is important evidence, but it is not a lifetime safety guarantee.

How the FDAโ€™s Position Has Changed

The regulatory picture has shifted as new evidence has become available.

In February 2025, the FDA removed language from the boxed warning that described an increased risk of adverse cardiovascular outcomes across testosterone products. The agency based that decision largely on the TRAVERSE findings.

At the same time, the FDA required class-wide warnings about increased blood pressure. Postmarket ambulatory blood pressure studies found that testosterone products could raise blood pressure across different formulations.

In June 2026, the Department of Health and Human Services announced that the FDA was requesting additional testosterone-labeling revisions. The proposed changes included removing the limitation stating that safety and effectiveness had not been established for age-related hypogonadism, revising prostate cancer information, and updating warnings involving benign prostatic hyperplasia.

Those June 2026 changes were requested updates at the time of publication, not proof that every previous concern has disappeared.

The regulatory trend reflects a more nuanced evidence base. Major cardiovascular and prostate fears appear less supported than they once did, while blood pressure and patient-specific risks still require attention.

Hematocrit Remains One of the Clearest TRT Risks

Testosterone stimulates red blood cell production. For some patients, this raises hematocrit, the percentage of blood made up of red blood cells.

A moderate increase may be expected. A larger or persistent increase can become clinically concerning and may require a dose adjustment, formulation change, temporary pause, or evaluation for contributing factors.

Smoking, dehydration, altitude, lung disease, and untreated obstructive sleep apnea may also affect hematocrit. Injectable testosterone can produce larger fluctuations and may be more likely to raise hematocrit in some patients than certain other formulations.

This is why monitoring testosterone therapy cannot stop once the initial prescription is issued.

Clinical guidance recommends checking hematocrit before treatment, repeating it after therapy begins, and continuing periodic monitoring. Blood donation or therapeutic phlebotomy may be considered in selected cases, but repeatedly removing blood should not become a substitute for reviewing whether the testosterone protocol itself is appropriate.

Blood Pressure Deserves More Attention

Blood pressure receives less attention in many TRT discussions than heart attack or stroke, but current regulatory guidance makes it an important part of long-term monitoring.

A small increase may matter more in a patient who already has hypertension, kidney disease, sleep apnea, diabetes, or a high baseline cardiovascular risk. The effect may also be easy to miss because high blood pressure frequently causes no obvious symptoms.

A responsible clinic should review baseline cardiovascular history and encourage regular blood pressure checks. If blood pressure rises after treatment begins, the provider may need to review the dose, formulation, other medications, fluid retention, sleep quality, and overall risk profile.

The purpose of monitoring is not simply to confirm that testosterone increased. It is to determine whether treatment remains medically reasonable as the patientโ€™s health changes.

What Research Says About Prostate Safety

The belief that testosterone automatically causes prostate cancer has not been supported by modern clinical trials.

A prostate-safety analysis from the TRAVERSE program found low rates of prostate cancer and other major prostate events. Rates did not differ significantly between the testosterone and placebo groups during the study period.

The 2026 HHS announcement similarly stated that available clinical and epidemiologic evidence had not generally demonstrated an increased prostate cancer risk among men receiving TRT. It also noted that existing studies may not be long enough to detect cancers that develop over much longer periods.

That limitation matters.

The TRAVERSE study excluded some men at high prostate risk, and follow-up lasted years rather than decades. The findings should not be interpreted as permission to ignore prostate screening or new urinary symptoms.

Providers may evaluate prostate-specific antigen, age, family history, urinary symptoms, prior test results, and other individual risk factors before and during therapy. A changing PSA result does not automatically mean cancer, but it may require additional evaluation.

Safe TRT Is a Process, Not a Product

The same medication can produce very different risk profiles depending on how it is prescribed and followed.

A patient who has relevant symptoms, consistently low morning testosterone levels, appropriate baseline testing, conservative dosing, and ongoing follow-up is not in the same situation as someone using testosterone without a confirmed diagnosis or clinical monitoring.

Good care includes a review of symptoms, medical history, medications, cardiovascular risk, prostate considerations, fertility goals, sleep apnea risk, and baseline laboratory results.

After treatment begins, the provider should assess whether symptoms are improving and whether the protocol is creating new problems. Follow-up may include testosterone levels, complete blood count or hematocrit, blood pressure, PSA when appropriate, estradiol when clinically indicated, and other tests based on the patientโ€™s health.

These are the practical foundations of approaches to safe, monitored TRT. Technology and telehealth can make follow-up more convenient, but the clinical standard should remain the same.

Fertility Is a Major Long-Term Consideration

TRT can suppress the signals that support natural testosterone and sperm production in the testes.

Some men experience substantial reductions in sperm count. Testicular size or fullness may also decrease. Recovery after stopping therapy can occur, but the timing and degree of recovery vary.

Men who want children soon, or who may want them later, should discuss fertility before starting testosterone. Options may include baseline semen analysis, sperm banking, avoiding TRT, or considering other clinically appropriate approaches.

Adjunctive medications are sometimes discussed, but none should be presented as a guaranteed way to preserve fertility. Reproductive goals require individual evaluation and may justify referral to a reproductive specialist.

Other Risks Depend on the Patient

TRT may worsen untreated severe obstructive sleep apnea in susceptible patients. It can also contribute to acne, oily skin, breast tenderness, fluid retention, hair loss in genetically predisposed men, and mood changes.

Some patients experience changes in estradiol as testosterone is converted through aromatization. An elevated number alone does not always require medication. Providers should consider symptoms, dose, timing, and the broader clinical picture before adding an aromatase inhibitor.

Treatment can also affect cholesterol markers differently across patients and formulations. These changes reinforce the need to assess overall health rather than focusing only on testosterone levels.

TRT Is Not the Same as Unsupervised Steroid Use

One persistent TRT myth is that medically supervised testosterone therapy and nonmedical anabolic steroid use carry the same risk profile.

They do not represent the same exposure.

TRT aims to restore testosterone to an appropriate physiologic range in a patient with a medical indication. Nonmedical anabolic use may involve much higher doses, multiple substances, uncertain product quality, and limited or nonexistent clinical monitoring.

That does not make TRT risk-free. It means findings from extreme anabolic steroid use should not automatically be applied to monitored replacement therapy, and TRT research should not be used to claim that higher-dose performance use is safe.

Oxandrolone, commonly known by the brand name Anavar, is also a Schedule III controlled anabolic steroid with specific medical indications and potential liver, lipid, hormonal, and cardiovascular concerns. What the Research Still Cannot Promise

Current evidence is reassuring in several important areas, but it does not support the claim that every man can use TRT safely for life.

Many major trials enrolled middle-aged and older men, so their findings may not apply fully to younger patients beginning decades of treatment. Trial participants are screened and monitored more closely than many patients in routine practice. Some high-risk groups are excluded from studies altogether.

Research also cannot determine whether TRT is appropriate based on symptoms alone. Fatigue, low libido, mood changes, weight gain, and reduced performance can come from sleep disorders, thyroid disease, depression, medication effects, metabolic conditions, or other causes.

The safest decision begins with an accurate diagnosis.

The Bottom Line on TRT Long-Term Safety

TRT is not the cardiovascular threat that older warnings sometimes implied. Large modern research has not found an increase in major heart attacks, strokes, or cardiovascular deaths among appropriately selected men over several years of treatment.

That does not make monitoring optional.

Blood pressure, hematocrit, prostate health, fertility, sleep, symptoms, and treatment response still matter. Some risks are predictable. Others remain uncertain because even the best current studies do not cover several decades of use.
